Thank you for being on Theta Provider Capacitation Programme. The training sessions have been completed and it’s time to submit your hardcopy file of your company’s complete Accreditation PORTFOLIO OF EVIDENCE. Attached for your information is the blueprint that you need to utilise as a template to assist you in compiling your final accreditation file. Please ensure that you compile your Accreditation POE utilising the supplied template as well as the THETA PHASE B application that can be found on the THETA website: www.theta.org.za. Kindly be advised that although you have attended different training session on the programme, all your company’s relevant information as required by the THETA PHASE B application must be submitted with your PORTFOLIO OF EVIDENCE. Guidelines to compiling your Accreditation POE: 1. Use the blueprint provided with the Theta Phase B application form.
